Health Coaching Agreement

Thank you for choosing to work with me. This document brings together everything you need to know about how coaching works, what you can expect from me, and the important information I need to share with you.

By booking and paying for your first session, you confirm that you have read and agreed to the terms below.

1. Purpose of Coaching

Health coaching is a supportive, collaborative process where we explore your health and wellbeing goals and create realistic steps towards positive change. I provide guidance, encouragement, and accountability - but you remain in charge of your own choices.

2. My Role

  • I am a qualified health coach and a registered member of the UK & International Health Coaching Association (UKIHCA).

  • I hold professional indemnity and public liability insurance with Balens.

  • My role is to support, guide, and empower you - not to diagnose, prescribe, or treat medical conditions.

3. Disclaimer

  • I am not a doctor, therapist, or dietitian.

  • Coaching is not a substitute for medical care.

  • I will not prescribe medication or offer medical treatment.

  • If you have or suspect a medical condition, you should consult your GP or another qualified healthcare professional before making changes to your diet, exercise, or lifestyle.

  • All decisions you make following our sessions are your responsibility.

4. Informed Consent

By working with me, you acknowledge and agree that:

  • You understand the role and limits of health coaching.

  • You accept full responsibility for your own health, wellbeing, and lifestyle choices.

  • You will let me know about any relevant health conditions or medical advice you have received.

  • You understand that results cannot be guaranteed, as outcomes depend on your own effort and commitment.

5. Session Arrangements

  • Format: Sessions may take place in person, by phone, or online.

  • Length: Each session lasts up to 60 minutes.

  • Frequency: You are free to choose how often you’d like sessions (weekly, fortnightly, monthly, or as needed).

  • Booking: Sessions must be scheduled in advance. (www.laurawilliamscoaching.com)

6. Fees & Payment

  • Fees are £60 per session.

  • Payment is due at the time of booking.

  • Payments can be made via online payment system at time of booking (www.laurawilliamscoaching.com)

7. Cancellations & Changes

  • Please give at least 24 hours’ notice if you need to cancel or reschedule.

  • Cancellations made with less than 24 hours’ notice may be charged in full.

  • Refunds are not provided for unused or missed sessions unless otherwise agreed.

8. Confidentiality & Data Protection

  • Everything you share in coaching sessions is kept private, unless I am legally required to disclose information (e.g. risk of serious harm).

  • In line with UK GDPR and the Data Protection Act 2018:

    • I will store your personal information securely and use it only for coaching purposes.

    • I will not share your details with third parties without your consent, unless legally required.

    • You have the right to access the information I hold about you, and to request corrections or deletion where appropriate.

  • I will keep client records securely for up to 7 years, after which they will be deleted or destroyed.

9. Ending Coaching

  • You may stop coaching at any time.

  • I may also end our coaching relationship if I feel it is no longer appropriate or beneficial.

  • If you have prepaid for sessions, we will agree a fair arrangement regarding any unused sessions.

10. Limitation of Liability

I will always do my best to support you. However, you understand and agree that I cannot be held responsible for your decisions, actions, or outcomes, and that I do not guarantee specific results.

11. Governing Law

This agreement is governed by the laws of England and Wales (or Scotland/Northern Ireland, depending on your location).

12. Agreement

Health Coach Supervision Agreement

Thank you for choosing to work with me in a supervision capacity. This document outlines how supervision works, what you can expect from me, and the terms of our professional relationship.

By booking and paying for your first supervision session, you confirm that you have read and agreed to the terms below.

1. Purpose of Supervision

Health coach supervision is a collaborative, reflective process designed to support you in your professional development, ethical practice, confidence, and effectiveness as a health coach.

Supervision provides a structured space to:

  • Reflect on your client work

  • Develop your coaching skills

  • Explore ethical considerations and boundaries

  • Strengthen your professional judgement

  • Support your wellbeing as a practitioner

Supervision is not therapy, mentoring in business growth (unless explicitly agreed), or clinical case management.

2. My Role

  • I am a qualified health coach and supervisor (in-training).

  • I am a registered member of the UK & International Health Coaching Association (UKIHCA).

  • I hold professional indemnity and public liability insurance with Balens.

  • My role is to support, guide, and challenge you in your professional development.

  • I will provide a safe, respectful, and confidential space for reflective practice.

I will not take responsibility for your client work, business decisions, or professional actions.

3. Professional Responsibility

You remain fully responsible for:

  • Your client relationships

  • Your coaching decisions and actions

  • Maintaining appropriate professional boundaries

  • Working within your scope of practice

  • Ensuring you have appropriate insurance and professional membership

Supervision supports your practice but does not replace your professional accountability.

4. Confidentiality

  • Everything discussed in supervision is confidential.

  • You must anonymise client information when discussing cases.

  • I may break confidentiality only if legally required to do so (e.g. risk of serious harm, safeguarding concerns, or legal obligation).

Supervision discussions must not be recorded without explicit mutual consent.

5. Informed Consent

By working with me, you acknowledge and agree that:

  • You understand the purpose and limits of supervision.

  • You remain professionally and legally responsible for your client work.

  • You will bring honest and accurate reflections of your work to supervision.

  • Supervision outcomes depend on your openness, engagement, and implementation.

  • Results in your business or client outcomes cannot be guaranteed.

6. Session Arrangements

  • Format: Sessions may take place online, by phone, or in person.

  • Length: Each session lasts up to 60 minutes (unless otherwise agreed).

  • Frequency: You may choose monthly, bi-monthly, quarterly, or ad-hoc supervision.

  • Booking: Sessions must be scheduled in advance (www.laurawilliamscoaching.com).

7. Fees & Payment

  • Fees are £60 per 1:1 supervision session. Group session fees available on request.

  • Payment is due at the time of booking.

  • Payments can be made via the online booking system (www.laurawilliamscoaching.com).

8. Cancellations & Changes

  • At least 24 hours’ notice is required to cancel or reschedule.

  • Sessions cancelled with less than 24 hours’ notice may be charged in full.

  • Refunds are not provided for missed or unused sessions unless otherwise agreed.

9. Records & Data Protection

  • Brief supervision notes may be kept for professional and insurance purposes.

  • In line with UK GDPR and the Data Protection Act 2018:

    • Your personal information will be stored securely.

    • Information will be used only for supervision purposes.

    • Your details will not be shared without consent unless legally required.

    • You have the right to access, amend, or request deletion of your data where appropriate.

  • Records will be kept securely for up to 7 years, then deleted or destroyed.

  • You may request a copy of my Privacy Policy at any time.

10. Ending Supervision

  • You may end supervision at any time.

  • I may end supervision if I believe the relationship is no longer appropriate, ethical, or beneficial.

  • If sessions have been prepaid, we will agree a fair arrangement regarding unused sessions.

11. Limitation of Liability

I will always act with professionalism and integrity. However, you understand and agree that:

  • You remain fully responsible for your professional practice and decisions.

  • I cannot be held liable for your client outcomes, business results, or professional actions.

  • No specific outcomes or results are guaranteed through supervision.

12. Governing Law

This agreement is governed by the laws of England and Wales (or Scotland/Northern Ireland, depending on your location).

13. Agreement

By booking and paying for supervision sessions, you confirm that you have read, understood, and agreed to this Health Coach Supervision Agreement, including confidentiality, professional responsibility, and data protection terms.
